The talk described how the KRAS-G12D mutation drives abnormal myeloid development in MDS and AML, causing immature blast expansion, treatment resistance, cytopenias, and poor survival. Using combined transcriptomic and genotyping analysis (GOT), the speaker showed KRAS-mutant AML cells were enriched for inflammatory, TNF/NF-kappaB, and CD83-associated programs. CD83 marked inflammatory, stem-like mutant cells and was elevated in patient samples and a mouse model. KRAS inhibition reduced mutant clones, inflammatory genes, monocytes, and tumor burden while restoring erythroid differentiation. The study suggests KRAS-G12D is an early driver of inflammatory myeloid disease and may be therapeutically targetable.
